Welcome to the 2008-2011 grant reporting page. Each UWSL grantee is required to submit semi-annual reports, due approximately 30 days after the end of each six month reporting period (i.e. January 31 and July 31). INSTRUCTIONS: The online software used for collecting your reports has been modified to allow you to save, retrieve and update your report information. To log in to your program's report form, you must have a "Program ID" number and password. Each program has a separate, unique "Program ID" that was provided during orientation meetings or via email. The initial password is the same for each program, but you can change the password once you log in. Please be sure to keep a record or each program ID and password! The online report form will display the indicators that you have agreed to measure for your specific program(s). If you find that the indicators shown are not those that you agreed to, or are not the measures that best describe the impact of your program, please contact us ASAP so we can discuss corrections to this problem. Please read all instructions prior to completing the form. Please remember that the reporting form is a PROGRAM reporting form, and must be completed separately for each funded program. IMPORTANT: Certain documents will need to be delivered in hard copy to UWSL by noon on February 1st (rather than being uploaded electronically). As per the contract, please submit 3 paper copies of your audit and IRS form 990 if your fiscal year ended June 30 or September 30; and your Utah State Charitable Permit if it was renewed between August 1 and January 31. Many of you have submitted these already.
